Tooth decay remains the most common chronic disease affecting young children in the United States, with more than 40% of children under six experiencing cavities. Traditionally, dentists have treated these cavities with drilling, injections, and, in severe cases, surgery under general anesthesia.
Now, researchers have reported encouraging results from a major clinical trial showing that a drill-free cavity treatment could transform pediatric dental care. The findings, published in JAMA Pediatrics, reveal that Silver Diamine Fluoride (SDF) can stop the progression of cavities in baby teeth without drills, injections, or sedation.
What Is the Drill-Free Cavity Treatment?
Silver Diamine Fluoride, commonly known as SDF, is a topical liquid that dentists apply directly to decayed teeth using a small brush.
The treatment combines two active ingredients:
- Silver, which kills cavity-causing bacteria and prevents further bacterial growth.
- Fluoride, which strengthens weakened tooth enamel and helps slow or stop decay.
Unlike conventional fillings, SDF does not require removing damaged tooth material with a dental drill. Each application takes only a few seconds.
Clinical Trial Shows Promising Results
Researchers from the University of Michigan led a Phase III clinical trial involving 830 children younger than six years old across research centers in Michigan, New York, and Iowa.
The project received more than $12 million in funding from the National Institute of Dental and Craniofacial Research (NIDCR).
The study found several important benefits:
- More than half of treated baby teeth showed arrested cavity progression.
- Dentists safely treated children as young as one year old.

Experts believe SDF could significantly change the way dentists and pediatric healthcare providers manage early childhood tooth decay.
The treatment may allow doctors to identify and manage cavities during routine health visits before serious complications develop. It could also benefit elderly patients, individuals with special needs, and people with severe dental anxiety.
If regulators approve SDF specifically for cavity treatment, insurance coverage could expand, making the therapy more affordable and accessible for families.
